病情證明書怎麼寫【篇一：病情證明書——】門診號病情證明書醫師簽名備註：到一樓門診部、新農合辦公室或放療樓二樓醫務科蓋章。【篇二：醫院病情證明書管理制度】醫院病情證明書管理制度病情證明書是具有一定法律效用的醫療檔案，是作為司法鑑定、保險索賠、休假等重要依據之一。為做好此項工作、進一步加強病情證明書的管理，根據《中華人民共和國執業醫師法》及《醫療機構病歷管理規定》，結合我院實際情況，特作如下規定：1、病情證明書用於門（急）診及出院病人的病情、診斷、休假證明，由經治醫師開具並簽字，非經治醫師及無處方權醫師無權出具。2、凡需出具疾病證明書的患者，由經治醫師核對其身份，根據病情開具相關證明，字跡清楚、內容準確，不得塗改，不得弄虛作假；經治醫師簽字後，在門診導醫臺處蓋章生效。3、臨床醫師要以科學、嚴謹、實事求是的態度，認真開具診斷證明書和病假證明書，每項診斷都應具備客觀、科學的診斷依據，經治醫師對所做的診斷負責。4、開具病假天數為3天（婦產科人工流產及骨科部分病種除外）。期滿仍需繼續休息者，應在門診隨診後由接診醫師重新出具。5、病情證明書上只能寫病情、診斷及與病情診斷相關的醫囑、建議。我院診斷證明書不涉及職業病的診斷和病人傷殘情況、勞動能力（病退）判定。6、導醫臺須對醫師開具的疾病診斷書和病假證明書認真稽核，嚴格把關，遇有異議，可請示門診部主任或醫務科科長決定。7、導醫臺應加強對印章的管理
